For the sake of those who may be newer to the game, I will argue differently. Experience has proven to me mastered spells do fail. It may only happen once every thousand hours of play time, but I assure you, they will occasionally fail.

If nothing else, mastered spells can still be thwarted by saving throws.

It all depends on what you call a failed spell. I like to use the definition of using normal mana since you casted it correctly, or using half mana, because you failed to cast the spell correctly. In this definition, once mastered, you will always cast the spell correctly; but it can fail do to deafness, saving throws, etc.

I logged on and started casting cure light upon myself. This was my 382nd consecutive cast barring human error on the counting. I only took breaks to sleep for mana. I was suffering from no negative status effects at the time of this test. I had 0 MR at the time of this test.

It can fail to cast. I have seen it once or twice out of 10+ years. I was surprised when it happened, hit 'spells', and it said mastered. Prior to that I was as adamant as anyone.

I don't know. It takes me less than a percent to successfully cast a cure light spell, and I did not keep track of how frequently I lost mana during this test. It may be possible to track in a future test, but I wasn't paying any attention to my mana at this time.
