I think it all depends on the situation, honestly. I mean if you have two people in a room full of people, that hate each and one introduces themselves to the room, and uses greet all the other one shouldn't have to be obligated to greet them back.
Or like how it played out with Azo and May. It was an oversight like you said, but it turned into an interesting RP in the end.
Plus greeting and being greeted by every person that you interact with can hurt some players in the long run. Especially when you interact with a lot of lower level characters that don't hang around, and your character forgets a person that is more useful.
I just don't think everything has to be so black and white. I mean yeah it's the polite thing to do most of the time. But not all of the time.
