I was seeing both invisible and hidden and he didn't have anything camoed behind him. I even left the room with the guardian and it still didn't work if he had been in the way at all.
I have also noticed while attacking NPCs from the third rank, you cannot hamstring or circle them afterwards. Even though they aren't in meelee with you, but they still act as if they are fighting you.
