okay, theres two types of fear, room affecting, and group affecting.

if you target at group, then you only hit that group

if you cast it with no target, then it hits the entire room.

now if you use an item...

then that item works like you cast it on the room, however, the item has no concentration; so it will hit everyone in the room but it doesnt hold because items cant concentrate; so after it hits the room NPCs and players will run away, but NPCs will run back in via the track code, players will have to willfully reenter the room.

To my knowledge:

When you cast fear as a room effect, nothing happens let alone the echo only the caster gets.

One and a half year back, I used a wand to produce the fear effect and people affected by it burdened my concentration. It is changed now as it seems or I remember wrong.

I believe that the spell went ahead and affected even the people that followed the first victim out of the room. It just doesn't show on this log because at that point you can't see them. Some of them might have resisted it and some might not depending on their saving throw. Please let me know if you are sure that it doesn't work that way.
