Ah it sounds like two different NPC saw the killing blow from different rooms. That must circumvent whatever preventative measures d has in place to prevent duplicate reporting. It's testable.

That makes sense. Ralina killed Aldric from a room away with a xbow in front of several NPCs, and wound up wanted for something like 8 counts of murder.

I think Jhorleb is right that this bug might have to do with ranged combat.
