There are a number of reasons this might be the case.
A Member of a tribunal.
B Member of a cabal with cabal spell.
C Bug.
D Feature of B cord.

Bug. I have reported a similar incident when I had my necro and Destrivai has ambushed me, but failed to kill me.

No attempted murder in my outlaw memory at wilderness ranged attacks.

I've sent logs in to be looked at with the typo command when I've encountered silly things like that.
