I am not sure about this because it has only happened to me on a few of the NPCs in the game. I am sure its not a failure of the skill check because i can look at other things or NPCs and not leave hide.

This small man seems to be robustly built despite his diminutive stature.
From head to toe he is draped in a thick and dark velvet like material as
well as a few silver trinkets. A dark grey pallor covers the parts of his
body that aren't covered and even seems to spread into his scalp and facial
hair. From the looks of this man, he appears to have spent a great deal of
time below ground.
Overall, a dwarven acolyte is in excellent condition.

A dwarven acolyte is using:
<worn as armor> light chain mail (brand new)

The fur of this black horse is shiny and well-groomed. Its sleek form
bristles with power and energy, and it is obvious that this steed was bred
for battle. Its stance is beautiful and its attention is focused highly on
all that surrounds it.
Overall, a large, black horse is in excellent condition.

A battered and blighted road covered in muck
This once somewhat useful path has now become more trouble than it was
ever worth; the holes in the cobblestone allowing stagnant swamp water to
permeate through the dirt. It looks as if any step upon the sludge could
cause the valiant stones hanging on for cohesion to finally give way to the
sands of time. The decaying plant life mixed with soupy mud upon a steep
downward slope make this a dangerous terrain to be walking on.
It's partly cloudy, very hot, and breezy.
Obvious exits: n s
One lonely copper coin is here.
A few silver coins are here.
A stumpy male halfling is here, covered in fern debris.
A large, black horse walks in from the north.

l stumpy
Before you is a small man standing a bit over four feet tall. Covered in
plant matter and bundles of cypress branchs, he looks quite out of place in
a stone enviroment. With the size of his appendeges looking peculiarly
small compared to the rest of his body this wee person would easily be
described as stumpy.
Overall, a stumpy male halfling is in excellent condition.

I think it might have something to do with the person you're looking at and their int level. The smarter they are, the better chance they're going to see you moving through the shadows.

OMG, finally I can post! ITS A SPELL CALLED DETECT HIDDEN! SOME NPCs HAVE IT! IF YOU ARE HIDDEN AND YOU LOOK AT SOMEONE WHILE DETECTING! THEY SEE YOU! YOU ARE NO LONGER HIDDEN!
