While I do have no-spam on, that ghoul was in the second row and had no way of hitting him that round...

Did Xantium have a reaching weapon? Maybe the ghoul parried his attack. Or maybe it did a circle stab that was blocked or deflected?

They don't circle stab without orders, but the parry is possible and would explain it.
