This was the result of two midnight guards spawning in response to a group of three waltzing through Menegroth. I thought that NPCs double-cleaving was a thing of the past - but apparently not.
I have an idea of why this happened - and if it is indeed the case, it can be reliably replicated. I think it happened because a non-outlaw was standing in front of me with another outlaw (the leader) behind me in third row. Perhaps, in an attempt to strike the leader (who entered the room first) they automatically revert to cleaving the guy just in front of that outlaw (Oios) and then proceeding to do the ordinary cleaves on me afterwards. Perhaps a slight problem with the "sequence"? Like: check for outlaws;cleave outlaw1(leader);cleave outlaw2(Oios) - cleave outlaw1 then proceeds to fail because outlaw2 is in front and he is thus served four cleaves from two NPCs.
Now, that is just a proposal, but something is definitely amiss.
