From what I know of that situation and what I have seen in other bug posts, I think the problem was caused by mirror image shenanigans.

This was my rogue who obviously wasn't mirror imaged. And if he was you would have seen the 'the headbutt breaks your mirror image'.

Been like this for a long time, and posted in bug discussion before also. The damage check is separate from the knocked unconscious check, because headbutt does very little damage in and of itself.

So if a pc misses a headbutt and does no damage it might still knock an npc or pc unconsoiucs...out?

The miss line is just indicating the amount of damage. He failed the save to not get knocked out.

Trust me, that is also true to life. I have been knocked out by things I did not feel. Its like a time/space rift thing. The force of the blow that knocked you out ripped a hole in the time/space continum and knocked you out before you felt the pain. And then, really, you felt the pain, or at least you would have but you were knocked out.

Its really just simple physics. Then again, I'm no doctor, or anything.
