I've only observed this once since the knockout rate is not very reliable:

If you open combat with headbutt and are in mode stun, your target will be knocked out and combat will not initiate. However, your target is not treated as stunned so you can't take items from them.

This seems like it probably comes from some recent changes to how stun and headbutt KO work. I opened combat with a headbutt KO while on mode kill once and it seems to initiate combat normally.

Actually, it's correct that KO'd people are not considered to be stunned. I actually changed the text string for head butt in the last major update so that it says 'knocked senseless' instead of using the word 'stunned', to eliminate this exact confusion.

Thanks for the response. I get that headbutt KO isn't a stun anymore, but it was just odd that combat stops, yet the target isn't stunned yet. I guess it's not a big deal either way.
