In terms of getting quality responses, the solution might be to do all of what you're suggesting, but with some of it in a different medium.

If a player doesn't roll a new character within a certain timeframe, the system could be aware of this and know how to e-mail an "exit interview" survey to the player's e-mail address.

This would give you data outside the immediate crisis of deletion for non-returning, which is probably for the best.

The list was derived through a brainstorming and poll on the forums long ago. If you have some specific reason that you think would apply to a lot of people, then feel free to post here and I will consider adding it.

For the original poster, I still think #3 is the correct choice. The options were intended to be used as broadly as possible without overlapping. If you are too lazy to give it some thought, then it won't matter how many options there are. In fact, there are probably too many as it is, which was one of the reason we put a lot of thought into the original choices.

It definitely would not be an improvement to make it a questionnaire. If you really want to give additional opinions, use the staff contact info or go to town in your Afterlife thread.

That line could be added as a general piece of info below the list of options right along with a "None of the Above" option. It would improve the quality of your data collection for not much work.
