I have read the help files many time and was wondering it this was a bug
I am currently using a double-headed hand axe (one handed) the one you get in Nerina and I am 100% certain that I have never had a third attack from it. I was wondering if this is a bug or just the axe is slow.

It is size giant if it makes any difference

Probably just slow. You do have third attack trained, right?

Weapon speed of the weapon determines which extra attacks it is eligible to use. Most axes are too slow to get third attack, but they do quite a bit more damage per hit than weapons that are quicker.

That weapon type is battleaxe, which has 1-2 attacks and low accuracy as well.

Axes are still useless comparing to other weapon types after the weapon twinking-wimping.
