is this something to expect to be happening frequently, or is it just certain pieces, or what? Also is there a way to tell whether something's going to do this or is it just gonna be trial and error?

Certain peices have whats called Deathrot, they rot a little while after you kill the NPC, there are ways to get around it I think, they might have changed it though.
