A mud I used to play had an RPsheet that was once required before advancing to level 15 and beyond. This has since been lifted and while not a necessity, there is a likely chance that a death occurred could cause a permanent death without reimbursement. The sheet consisted of five areas: Goals, Personality, Background, Characteristics and Current. This is from their helpfiles.

Goals:
These are your characters goals in life. There could be one or more long
term goals, split into subgoals on how your character hopes they will
accomplish those goals. (minimum of 5 lines)
Characteristics:
These are things about your character that help define them. This could
be anything from never touching silver coins due to a belief that they're
all cursed to having an unusual birthmark. Again, a character is bound
to have multiple things that can be explored here. (minimum of 10 lines)
Personality:
These are your characters personal traits. Is the character mean-spirited
and take everything they want? Most characters will have several traits.
It may help to choose a person you know well and to think about their
personality; that may give you some ideas of how you can explore what
traits you want your character to have. (minimum of 10 lines)
Background:
These are the events leading up to the current. This may start somewhere
before the characters birth if appropriate. It should include things that
happened in the character's childhood, adolescence, etc. Things that have
made the character who they are today.
Current:
Occasionally something of interest will happen to your character that
will cause it to change in fundamental ways. For instance, if you said
in your Personality section that your character was very childlike, but
then saw your best friend killed brutally, it might cause you to change
your outlook on life. Once the RPSHEET is locked, this is the only way
of editing or updating it. It is not required that this field be filled
before the RPSHEET is locked.
I'm not saying that it has to go as elaborate as this, but I've always like the idea of it. For those who don't like that, you'd have LOVED the 40 line requirement story posted to everyone as to the who, what, when, where and whys of PKing someone, everytime.
