I noticed this to happen a lot with casting NPCs.

[Skills help] combat casting

Automatic

Paladins and hellions are unique in that they are highly trained in
both the martial and mystic arts.  This rigorous training allows them to
perform a feat that only they can - the limited ability to fight with
their weapon at the same time that they concentrate on and cast spells.

Combat casting provides two distinct benefits.  The first is the
ability to attack once per round while casting.  This style of fighting
is incredibly difficult - even with special training it is impossible
to make multiple attacks per round while casting a spell.  For this
reason many combat casters use the largest weapons available.

The second benefit is the ability to suspend the casting of a spell in
the event that they are knocked to the ground.  While bashed or tripped,
a combat caster will receive their full number of attacks per round, as
if they were not in the process of casting a spell.  Normal casting will
resume once they regain their footing.

I don't think that's what we're seeing in the log posted.

Actually, its just a normal hit from a NPC that does divine wrath attacks in the middle of the damage message for flamestrike.

Only ever happens with flamestrike. And yeah, I've seen it as well, doesn't appear to be reliably reproducable.
